What obsessions and compulsions can look like
Obsessions are recurring thoughts, images, or urges that are intrusive and distressing. Compulsions are repeated behaviors or mental acts performed to reduce distress or prevent a feared outcome. Common themes include contamination, harm, mistakes, morality, relationships, religion, health, sexual thoughts, symmetry, or the need for certainty.
- Washing, checking, repeating, arranging, counting, reviewing, or confessing.
- Seeking reassurance even after receiving a clear answer.
- Avoiding people, objects, places, information, or responsibility.
- Performing silent rituals, replacing thoughts, or mentally reviewing events.
- Spending substantial time on routines or becoming late because they must feel complete.
OCD is not the same as being organized
Preference, precision, or perfectionism alone does not establish OCD. The clinical question is whether intrusive experiences and rituals are time-consuming, distressing, or impairing. Children may have difficulty explaining why they perform a behavior. Some adults recognize that a fear is unlikely and still feel unable to resist the ritual.
What else the evaluation considers
Generalized anxiety, panic, depression, trauma-related symptoms, psychosis, autism-related repetitive behavior, tic disorders, body-focused repetitive behaviors, eating disorders, illness anxiety, ADHD, and personality traits may overlap with parts of the presentation. Medication effects, substances, sleep, medical history, developmental context, and suicide risk also matter.
Evidence-based treatment planning
Exposure and response prevention, commonly called ERP, is a specialized form of cognitive behavioral therapy with strong evidence for OCD. It involves planned contact with feared situations while reducing rituals and reassurance. Medication may also be considered, particularly when symptoms are moderate to severe or psychotherapy alone has not been sufficient. Renova Health can provide psychiatric evaluation and medication management and coordinate with or refer to an ERP-trained therapist when appropriate.
Medication for OCD requires adequate follow-up
Medication choice depends on age, medical history, other diagnoses, previous response, side effects, interactions, pregnancy considerations when relevant, and patient preference. Improvement may take time, and dose or duration requirements can differ from treatment for depression. Medication should not be started, stopped, or changed without the prescribing clinician.
How progress is measured
Useful outcomes include less time spent on rituals, reduced avoidance, improved school or work participation, greater tolerance of uncertainty, and better family functioning. Family accommodation and repeated reassurance may need to be addressed without blaming relatives who have been trying to help.

Are intrusive thoughts evidence that I will act on them?
Intrusive thoughts can be unwanted and inconsistent with a person's values. Risk still requires an individualized assessment, particularly when intent, planning, psychosis, or loss of control is present.
Does Renova Health provide ERP?
Renova Health evaluates OCD and provides psychiatric treatment planning and medication management. When structured ERP is indicated, the practice discusses coordination with an appropriately trained therapist.
Can I make an appointment without a referral?
Yes. Patients may request an appointment directly, although an insurance plan may have separate referral or authorization requirements.
